Initial New Patient Evaluation: $375 1.5 hours Diagnostic Revaluation (former patient returning to the practice): $250 1 hour Comprehensive forensic evaluations (for legal cases, Workman's compensation claims and disability lawsuits requiring complete record review and full report-not current patients) $500 for 2 hour evaluation and $250 for each additional hour as needed for report to be completed The initial diagnostic assessment is the starting point for psychiatric care. Accurate diagnosis defines treatment choices and treatment goals. It is not uncommon to have multiple disorders simultaneously contributing to distress and disability. The initial task of a diagnostic evaluation is to identify and characterize these disorders and how they contribute to the problems leading a person to seek psychiatric care. Once the nature of the problem has been identified it is possible to have a discussion of treatment options: what to treat, what not to treat, and in what order. The average time for the evaluation is 1 1/2 hours. Medication Follow-up: $135 Medication management visits have two functions: 1. Medication stabilization is the first phase of pharmacologic treatment. The goal of this phase is to work together to find the right medication or combination of medications, at the lowest effective dose, to treat identified problems warranting treatment. My role is to educate patients as to medication choices, possible side effects of those choices, and expected benefits. Every patient is potentially different in their attitudes,tolerance, and response to particular medications. My job in this phase is to understand these differences, respect each patient as an individual, and work together to achieve symptom remission. 2. The maintenance phase of treatment starts once symptom remission and medication stabilization is achieved. It is generally based on visits every three months. Prescriptions are refilled at this time, questions are answered, and level of functioning and sense of wellbeing are discussed. Medication adjustments may be necessary periodically and can be considered. The average time for a visit is 20- 30 minutes. Report writing: $250/hour Preparation of written reports or letters for disability, legal, or other purposes. Services provided at no additional charge: 1. On-call availability between appointments by telephone to briefly answer questions or solve medication related problems that cannot wait until the next scheduled appointment. 2. Medication call-in to the pharmacy if medications are lost or running too low to last until the next scheduled appointment. 3. Telephone communication with primary care providers, therapists, schools, or attorneys when necessary. 4. Preparation of a receipt that can be submitted by patients to insurance companies that offer out-of-network benefits. 5. Provide renewal prescriptions between appointments for those medications (i.e. controlled substances such as stimulant medications for Attention Deficit Hyperactivity Disorder) that cannot be refilled without a "hard copy" (i.e. paper prescription) or called in to the pharmacy.
